The association between female gender and the presence of facial lipodystrophy was evaluated in a cross-sectional analysis of 248 HIV-positive patients. Face photographs were taken of each patient, and the presence or absence of facial lipodystrophy was determined by one physician and confirmed by two other physicians. Patients were classified as having “facial lipodystrophy” if they were rated as having “any facial lipodystrophy” by two or more of three physicians. Univariate analysis demonstrated that female gender, advanced age, and use of protease inhibitor therapy were associated with the presence of facial lipodystrophy. For each variable, the prevalence of facial lipodystrophy increased with age. After adjusting for age, sex, duration of HIV infection, and use of protease inhibitor therapy, multivariate logistic regression analysis demonstrated that female gender (OR, 4.34; 95% CI, 1.29-14.63; P = 0.02), duration of HIV infection (OR, 1.10; 95% CI, 1.02-1.19; P = 0.01), and advanced age (OR, 1.09; 95% CI, 1.01-1.17; P = 0.03) were independently associated with the presence of facial lipodystrophy.
